Fertility Preservation Study

This is a non-interventional multicentric multinational study to assess different controlled ovarian stimulation protocols for oocytes/embryos cryopreservation in cancer patients.

Apart from question 7 which allows multiple answers all other questions require one entry as an answer.
If the same patient was treated with more than one stimulation cycle please separate each cycle in a different data entry.
